STEPS FOR OBTAINING A WORK PERMIT

1. Pick up an “Intent To Work” form from Morton High School.

2. Take the form to the place of future employment and have the employer sign it.

3. The parent and student must sign the form.

4. Take the signed form to the Scott Middle School counselor to check grades and attendance.

5. If it is acceptable, the counselor will sign the form.

6. Take the form to Morton High School with the student’s birth certificate and they will give the student a Work Permit.
