Sports Physicals For the 2012-2013 School Year

All incoming 6th-12th grade Hammond students are invited to attend one of four Sports Physical Clinics for only $10. There are no out- of-pocket costs to you if you bring your Medicaid or Private Insurance information.

Locations and Dates are as follows:

Clark MS/HS Tuesday April 24th 3:30-5:30PM

Hammond High School Tuesday May 1st 3:30-5:30PM

Gavit MS/HS Tuesday May 8th 3:30-5:30 PM

Morton HS Tuesday May 15th 3:30-5:30PM

 

All students must bring a completed Sports Physical form in order to be seen. Forms are available in your school Health Services office, in the main office, from the Athletic Director, or you can print one by clicking here and fill it out.

Immunizations required for 6th-12th grade students will also be available on-site. Please bring your child's shot records.

A parent/guardian must accompany all students not presently in high school.
